| This form is to assist you and us in quickly processing the contributions you make for your employees. The use of the form is not compulsory and we understand that this may not suit large employers, however for small to medium employers it may be useful. If you require further information or help with the spreadsheet you can call the employer helpline on 1800 636 441.
This spreadsheet has been locked against redesign to protect against accidental format changes. You may experience 'cut and paste' issues when copying locked cells into this spreadsheet. | Leave Without Pay - Retirement Scheme |
| This form is used to notify the scheme of a period of leave without pay for a Retirement Scheme member. Only periods greater than 5 days are to be reported. Only periods of leave that cover a whole month will have an impact on the payment of contributions to the scheme.

| Employment Termination Advice |
| This form is to be completed by the Employer to advise
the Scheme of a member’s termination of employment.

| Employer Statement |
| This form is to be completed by the Employer to enable the Scheme to assess a member's eligibility for a Partial and Permanent and/or Total and Permanent Invalidity benefit.

| Change in Hours Worked - Retirement Scheme |
| This form is used to advise a change in the basis of employment for a member of the Retirement Scheme i.e. Full-time to Part-time, Part-time to Full-time and Part-time to Part-time (different hours worked).

| Contribution Remittance Advice - Retirement Scheme |
| This form to be completed and attached to any cheque or paperwork sent to the Scheme. Payments will not be accepted if this form is not
completed and attached.

| Please note that this form should be completed by the member and submitted to their employer and not to the Energy Industries Superannuation Scheme

| Authorised Officer application to access employer online |
|
Only Authorised Officers should fill out this form. The definition of an Authorised Officer means an employee of the Employer who has
been authorised by the Chief Executive to establish Authorised Users of the service.
Authorised Users should not fill out this form but should fill out an “Authorised User application to access employer online” form.

| Secure Site Access Agreement |
|
This form needs to be completed by the Authorised Officer before they can gain access to the online system. |

|
Only Authorised Users should fill out this form. The definition of an “Authorised User” means an employee of the Employer who is
authorised under this agreement to use the service on behalf of the Employer.
Authorised Officers should not fill out this form but should fill out an “Authorised Officer application to access employer online” form. An Authorised Officer will need to sign this form. |

The purpose of this document is to define the Employer Online standard file formats that employers should be using to provide electronic files of member information for each scheme. Contribution files to be submitted should be sent in this format where possible or contained within the Contribution Spreadsheet (available for download above). |
